Individual health insurance typically covers birth control if a medical prescription is required (., birth control pill) and the woman is under twenty years of age. Once a woman is twenty, her health insurance does not cover any form of birth control unless it is considered medically necessary. Doctors prescribing birth control to girls under the age of fourteen require parental consent. Doctors may determine whether young women between fourteen and sixteen years old are mature enough and do not require parental consent for the prescription of birth control. Young women sixteen or older do not require parental consent for any form of birth control.

Provide examples of your co-worker's performance or behavior that suggests constructive feedback is warranted. If you're writing a paragraph based solely on your perception or personal opinion, the feedback has little credence and, therefore, probably won't appear to be written in good faith. Use objective, non-accusatory and neutral terms to describe areas for improvement. For example, if your co-worker's technology skills aren't up to par, explain the times she has come to you for help using software applications with which she should be familiar. Follow up with a summary of how you've coached her to be more efficient through using the appropriate programs.

What was reported in 1957 (and elsewhere afterwards) was the issue of certain radio stations banning the Elvis album from the airwaves, but these were reported at the time in newspapers and trade journals anyway, as I stated in the piece.  The Berlin element was not reported.   Perhaps, once upon a time, there was a story in an Elvis Monthly (or similar) that might have mentioned something, and if that's the case then we have to re-evaluate my findings, of course - providing the sources there seem legit rather than based on rumour.  But going by the substantial archives we have access to, that wasn't the case.
